Abstract
Objective: To analyze the clinical and neurological manifestations of ulnar nerve injuries (UNI), evaluate the effectiveness of various surgical reconstruction techniques, and determine the influence of injury characteristics, timing of surgery, and electrophysiological degeneration on functional recovery. A total of 103 patients aged 18–67 years with traumatic or compressive UNI were examined. Clinical, electrophysiological, ultrasound, and neuroimaging assessments were performed. Surgical interventions included primary and secondary neurorrhaphy, autologous nerve grafting, neurolysis, anterior transposition, and tendon transfers in chronic cases. Early reconstruction (<4 weeks) and grafts <4 cm demonstrated the highest functional recovery. Severe axonal degeneration significantly reduced intrinsic muscle restoration. Tendon transfers provided acceptable functional outcomes in late-stage injuries with irreversible denervation.
